We're looking for a compassionate, organized, and driven Targeted Case Manager (TCM) to join our COACHES and Community-Based Services team. If you're passionate about advocating for youth with behavioral health needs and enjoy working in the community, we'd love to meet you. What You'll Do As a Targeted Case Manager, you'll work directly with children experiencing serious emotional or behavioral challenges and their families to coordinate services that make a lasting difference. Your responsibilities include: Conduct comprehensive assessments and develop individualized service plans. Coordinate behavioral health, educational, medical, and community services. Connect families with resources and advocate for needed supports. Monitor progress and adjust service plans to ensure goals are being met. Participate in treatment team meetings and Medicaid staffings. Complete accurate documentation in accordance with Florida Medicaid requirements. Provide crisis support and help families navigate challenging situations. Build collaborative relationships with schools, providers, and community partners. Assist with discharge planning and continuity of care. Transport clients to appointments when necessary using a SAYS vehicle. QualificationsWhat We're Looking For Required Qualifications One of the following: Bachelor's degree in Social Work, Psychology, Counseling, Human Services, or a related field and at least one year of experience working with children experiencing serious emotional disturbances OR Bachelor's degree in a related field plus three years of qualifying experience working with children with serious emotional disturbances. Additionally, candidates must: Be eligible to obtain the Certified Behavioral Health Case Manager (CBHCM) credential within 30 days of hire. Complete required AHCA Targeted Case Management training within required timeframes. Possess knowledge of community resources and behavioral health services. Have excellent communication, documentation, and organizational skills. Maintain a valid Florida driver's license with an acceptable driving record. Why Join SAYS?
