Careage Home Health is seeking a Home Health Physical Therapist to join our team serving Thurston County. This position is a great opportunity to grow your PT skillset while providing essential one-on-one care to individuals in the home care setting. A Registered Physical Therapist (PT) administers physical therapy to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Director of Clinical Services.


Summary of Responsibilities:

  • Travel within Thurston County to provide care on an intermittent basis in patients' homes.
  • Provide physician orders and plan of care under the direction and supervision of the Director of Clinical Services.
  • Work in conjunction with all other home care personnel in patient care planning.
  • Direct and supervise Physical Therapy Assistant and Home Health Aide, as assigned.
  • Take initial patient history and make initial evaluation.
  • Consult with physicians regarding change in treatment.
  • Instruct patients and family in home programs and activities of daily living.
  • Participates in Quality Assessment and Performance Improvement activities as assigned.
  • Complete and submit OASIS assessments, reassessments, transfers, resumptions of care, discharges and significant change in condition.
  • Appropriately utilizes ICD-10 codes.


Requirements

  • Graduate from a physical therapist curriculum approved by the American Physical Therapy Association.
  • Currently licensed as a physical therapist in the state of Washington.
  • Two (2) years' experience as a physical therapist, preferred.
  • Home Health experience, preferred.
  • Detail oriented, self motivated and capable of efficient time management.
  • Critical thinker capable of working collaboratively with a team.
  • Valid WA State driver's license.
