Responsibilities

Coordinates with other departments to provide maximum level of services.  Must be able to demonstrate the knowledge and skill necessary to provide care, based on physical, psychosocial, educational and safety related criteria, appropriate to the age of the patients served in his / her assigned service area.  The skills and knowledge needed to provide such care may be gained through education, training and / or experience.  Responsible for providing administrative support and entry to CMS required patient data information into several systems such as CMS, UDS, SMS and CERNER.  Employee is also responsible for entering daily patient charges to ensure fiscal compliance. Employee shall also demonstrate accountability for and contribution to overall unit quality improvement, and all other duties as assignment to include assistance to unit staff with patient transport, transfers and translations.

Qualifications

Experience:  Minimum of two years clinical experience in hospital setting for Case Management.  Prefer Utilization Review / Discharge Planning background.

 

Licensure, Certification, Registration, etc.: Maintain current professional Texas RN licensure.

 

Special Skills, Knowledge:  Ability to work with multidisciplinary staff.  Working knowledge of current Medicaid / Medicare guidelines. 

 

Education:  Graduate from an accredited School of Registered Nursing.
