Home Care Transition Nurse-onsite-Farmingdale

The Home Care Transition Nurse is responsible for assessing, planning, and developing referrals.

• 5 x 8 hour shifts (9:30AM to 5:30PM) - Rotating Weekends and Holidays

• Additional compensation for visits over your caseload

This position is on site in our Farmingdale office located at 110 Bi-County Boulevard 

Our Home Care Transition Nurses:

• Establish and maintain professional relationships with key physician groups and referral sources.
• Act as Liaison to referral sources on behalf of Catholic Home Care.
• Identify potential market growth opportunities and program expansion.
• Serve as a Customer service representative and act in a professional and supportive manner at all times.

Requirements:

• Registered Nurse with current licensure in NYS 
• 2 or more years of home care experience preferred
• Customer Service and Marketing skills required
