Job Summary:
 Are you passionate about making a difference in the lives of individuals in their homes and community? We are seeking a Licensed Practical Nurse (LPN) for our Home Care team, working under the direction of the Nursing Manager and alongside RNs and the interdisciplinary team. In this role, you will provide direct nursing care in participants’ homes—including medication administration, wound care, lab draws, and respiratory treatments—while ensuring accurate documentation and participant safety.

As an LPN in Home Care, you will coordinate medication needs, perform medication reconciliations, manage weekly pre-fills and home deliveries, and support CNAs in the field. You’ll follow care plan interventions, communicate participant status changes to the team, and adhere to infection control and safety protocols. You’ll benefit from strong management support and a collaborative team culture that values participant-centered care.

This position offers a consistent Monday–Friday schedule, with only occasional rotating Saturday and holiday shifts—designed to support your work-life balance.
