Talbot Hospice is seeking an experienced and passionate Hospice Clinical Team Lead to provide leadership, oversight, and coordination of our hospice clinical services and intake operations. This  leadership role ensures patients and families receive exceptional, compassionate care while supporting staff development, quality outcomes, regulatory compliance, and operational excellence.


Key Responsibilities

  • Provide clinical leadership and oversight of hospice services.
  • Coordinate patient referrals, admissions, and intake operations.
  • Supervise and support clinical staff through coaching, mentoring, and performance feedback.
  • Facilitate interdisciplinary team (IDT) meetings and promote collaboration across disciplines.
  • Monitor quality measures, patient outcomes, and regulatory compliance.
  • Ensure appropriate staffing, scheduling, and productivity.
  • Assist with recruitment, onboarding, orientation, and retention of clinical team members.
  • Support quality improvement initiatives and foster a positive, patient-centered culture.
  • Participate in Clinical Manager On Call rotation.
  • Serve as a clinical resource for staff, patients, families, referral sources, and community partners.

Requirements

POSITION QUALIFICATIONS

  1. Registered nurse with current licensure to practice professional nursing in the state.
  2. Graduate of NLN accredited school of nursing with a BSN degree is preferred.
  3. Previous experience in hospice/home care setting and with two (2) years management or supervisory experience. Proven ability to work within an interdisciplinary setting.
  4. Complies with accepted professional standards and practice.
  5. Has excellent observation, good nursing judgment and communication skills. Understands hospice philosophy and issues of death/dying.
  6. Understands principles of pain/symptom management.
  7. Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
  8. Complies and maintains current CPR certification.
  9. Demonstrates excellent observation, verbal and written communication skills.
