Personal Care Coach Job Description

Job Overview: The Personal Care Coach is responsible for providing personalized care and support to individuals in the comfort of their own homes. This role ensures that clients receive assistance with daily tasks, promote independence, and ensure safety and well-being. This is a rewarding role that offers the opportunity to make a significant impact on our clients.

Responsibilities:

  • Personal Care Assistance:
    1. Support clients with the Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs) as described in the client care plan.
      • Bathing, grooming, dressing, meal prep, medication reminders, mobility support, etc.
      • If client requires transfer support, the Personal Care Coach will have the opportunity to learn how to use medical devices. 
    2. Provide a safe and healthy environment for the client.
    3. Maintain open communication with the family members in the home as requested.
    4. Communicate with the administrative team for any client care plan updates.
  • Incident Reporting:
    1. Notify QA before the end of the shift of any client incidents and document them in the client care portal.
    2. Notify HR of any caregiver incidents before the end of the shift and document them under WorkSafe in the client care portal.
  • Training:
    1. Complete trainings to maintain credentials (HHA/CNA/etc.).
    2. Complete trainings from Thrive (BASICS, etc.).

Skills/Qualifications:

  • Previous health care experience (1 year preferred)
  • CPR/First Aid (required)
  • Home Health Aide or Certified Nursing Assistant certification (preferred, not required)
  • Excellent communication and interpersonal skills.
  • Attention to detail and a commitment to continuous improvement.
  • Familiarity with relevant technology and software for documentation and reporting.
  • Must be able to lift up to 35 pounds.
  • Must be willing to train on medical devices to assist clients with transfers.

Technical Requirements:

  • Must own a smartphone with data.
  • Must own a personal vehicle with insurance that meets minimum state guidelines.
  • Must be willing to travel to all our service regions.
