Licensed Practical Nurse


Direct Resident Care

  • Provide routine bedside nursing care under RN and physician supervision.

  • Perform daily assessments, observe residents for changes in condition, and report findings promptly.

  • Assist residents with ADLs as needed (bathing, grooming, mobility, toileting).

  • Respond to call lights and resident needs in a timely manner.

Medication Administration

  • Administer medications safely and accurately per MAR, including oral, topical, inhaled, and certain injectable medications.

  • Monitor for side effects, adverse reactions, and effectiveness of medications.

  • Maintain accurate documentation of all medication administration and waste.

Treatments & Clinical Procedures

  • Perform wound care, dressing changes, and skin assessments.

  • Complete vital signs, blood glucose checks, and other routine clinical tasks.

  • Carry out physician-ordered treatments such as catheter care, ostomy care, nebulizer treatments, and tube feeding support.

  • Assist with lab draws and specimen collection.

Assessment & Monitoring

  • Monitor residents for changes in physical or mental status and escalate concerns.

  • Document assessments, interventions, and resident responses in EMR or charting system.

  • Participate in care planning by reporting resident progress and concerns to the interdisciplinary team.

Communication & Coordination

  • Communicate effectively with residents, families, physicians, RNs, CNAs, therapy staff, and other team members.

  • Participate in shift reports, relay changes in condition, and ensure smooth transitions of care.

  • Notify physicians and families of significant condition changes, incidents, or updates.

Supervision & Team Support

  • Direct and support CNAs, ensuring completion of daily tasks and quality resident care.

  • Assist with orientation and training of new staff as needed.

  • Promote teamwork, professionalism, and a positive care environment.

Safety & Compliance

  • Follow facility policies, infection control standards, and regulatory guidelines.

  • Participate in incident reporting, fall prevention, and emergency response.

  • Ensure safe medication practices and maintain proper storage of supplies.

Documentation

  • Complete timely, accurate, and legally compliant charting of all care provided.

  • Update care plans and nursing notes as needed.

  • Maintain accurate counts and records of controlled substances.


Requirements:
 - Must possess, as a minimum, a nursing degree from an accredited college or university.

- Must possess a valid and current LPN license in this state.
