ESSENTIAL FUNCTIONS: Provide comprehensive case management of complex, medical and/or behavioral health cases to include assessments, case planning, case plan implementation, monitoring and evaluation; Provide crisis intervention and supportive counseling to individuals and families; Link clients and families to services in the community for support such as job placement, debt counseling, legal aid, housing, medical treatment, or financial assistance; Serve as advocate between clients and schools, healthcare system, legal system, public assistance programs, and other social services agencies. Conduct case record review using specific indicators and criteria set forth by Social Services Department policy and regulating agencies; Maintain case files with timely record keeping and documentation as required by regulation and program policy; Conduct community trainings and presentations; Facilitate psychoeducation and support groups; Participate in professional development trainings; Assist in community service events and Emergency Preparedness; Respond to 24-hour emergency calls as assigned; Actively participate in weekly supervision with Coordinator and BHD Director; Other position related duties as required and assigned; QualificationsMINIMUM REQUIREMENTS: Masters in Social Work plus two years of experience; current licensure by the Texas State Board of Social Worker Examiners; Must have a current Texas Drivers License and be insurable. Must be able to successfully pass a post-offer drug screen. Must be able to successfully pass a background check. Ability to travel overnight (approximate 10 %.) Bilingual-English/Spanish. KNOWLEDGE, SKILLS, AND ABILITIES: Thorough knowledge of social work values, principles, techniques and practices and their application to casework, group work, and community needs; must be able to function as a member of an interdisciplinary group; must have excellent interpersonal skills. Must have working knowledge of Microsoft Office. Effective and efficient utilization of resources; excellent communication and listening skills. Must have good facilitation skills. Must be able to handle multiple priorities effectively. Ability to read, analyze and interpret technical procedures or governmental regulations. Must be able to communicate effectively with local, state and federal agencies. Ability to handle confidential information appropriately; Must have an understanding of the Tribal culture, practices and traditional form of government. Ability to effectively present information and respond to questions from client or Tribal Council. Attributes include diplomacy and professionalism. Must be able to work flexible hours as required. PHYSICAL REQUIREMENTS: While performing the essential functions of this position, the employee is frequently required to sit, talk, and hear. The employee is occasionally required to reach with hand and arms. The employee may be required to lift up to 40 pounds.
