As we modernize and standardize key systems across our portfolio, we are searching for an EHR Project Manager to lead organization-wide electronic health record (EHR) implementations, optimizations, and post–go-live integration efforts. This is a highly impactful role supporting our continued growth and ensuring our care teams have the technology they need to deliver high-quality service.

 

THE ROLE:

The company is seeking a highly organized, systems-minded Project Manager to lead complex, cross-functional technology and operational initiatives across our family of brands. Reporting to the Technical Program Manager, the Project Manager will oversee projects from strategy through execution, ensuring successful delivery of enterprise systems, integrations, and process improvements that support scalability, security, and operational efficiency. 

This role is expected to drive consistent, high-quality project execution across a complex, multi-entity environment while partnering closely with IT, Operations, and business stakeholders. This individual will play a critical role in building structure, governance, and best practices for how large-scale initiatives are planned and delivered as the organization continues to grow and modernize its technology platforms. 

You will be required to travel to one of our brand locations 1-2x month. Locations may vary.

 Primary Responsibilities:

  • Lead the planning, execution, and successful delivery of cross-functional projects spanning technology, operations, and enterprise systems 
  • Create and implement project runbooks and playbooks to enable consistent execution and scalable delivery models 
  • Translate business and operational requirements into structured project plans and scalable solutions 
  • Develop and manage project timelines, scope, resources, dependencies, risks, and supporting documentation. 
  • Serve as the primary point of coordination between internal teams and external vendors or implementation partners 
  • Facilitate requirements-gathering sessions and ensure alignment between technical solutions and operational workflows 
  • Partner closely with IT leadership and functional stakeholders to ensure solutions align with enterprise architecture and long-term strategy 
  • Manage project communications, including status updates, milestones, risks, and escalations to leadership 
  • Coordinate meetings, working sessions, and governance checkpoints across multiple stakeholder groups 
  • Support change management efforts through documentation, testing coordination, training readiness, and stakeholder adoption planning 
  • Ensure projects adhere to governance, security, and data integrity standards 
  • Identify opportunities for optimization, standardization, and continuous improvement across systems and processes 
  • Provide escalation support and issue resolution in partnership with leadership and project sponsors 
  • Support project activities that may require after-hours or weekend coordination to align with business operations and system deployment schedules 

 

WHAT YOU BRING TO THE TABLE:

Qualifications

  • Ability to work after hours or on the weekends when needed 
  • Bachelor’s degree and 4+ years of experience in project management, technical program management, or a hybrid business/technology role 
  • Technical background working knowledge of Microsoft, Entra: UI/end user perspective is highly preferred 
  • Experience supporting enterprise systems implementations or technology-driven transformation initiatives 
  • Working knowledge of modern enterprise platforms and identity, productivity, or collaboration tools from an end-user and business perspective (administrative or technical experience a plus) 
  • Proven ability to lead cross-functional teams through complex, multi-stakeholder projects 
  • Experience managing external vendors or implementation partners 
  • Strong technical aptitude with the ability to translate between business needs and technical solutions 
  • Healthcare, home care, disability services, or similarly regulated industry experience preferred but not required 
  • Experience supporting multi-entity or roll-up organizations strongly preferred 
  • Strong communication skills with the ability to engage stakeholders at all levels 
  • Highly organized, detail-oriented, and comfortable driving accountability 
  • Project management certifications (PMP, Agile, Scrum, or similar) preferred
