Job DetailsJob Location: Electra Home Health - Electra, TX 76360   Responsible for traveling to patient’s home to administer skilled nursing care on an intermittent basis. Monitors the patient’s overall health, and in partnership with a primary care physician, and develops an individualized plan of care for their patients.   ESSENTIAL DUTIES AND RESPONSIBILITIES: Responsible for the total nursing care, assessment, planning and implementation, and evaluation of each patient. Provide and coordinate nursing care by assessing patient’s nursing needs in accordance with the plan of care and within the standards and scope of nursing practice as defined within the state Nurse Practice Act regulations, policies, procedures, and protocols, as well as facility policies and procedures. Create the initial evaluation visit and ensure patients’ status is assessed on an ongoing basis and pertinent information is gathered and documented.  Develop the plan of care and necessary revisions. Accurately administer medications per physician’s ordered note. Provide appropriate preventive and rehabilitative nursing procedures.  Prepare clinical and progress notes for each patient visit and summaries of care conferences on patients in a timely manner as per Electra Home Health’s policy.  Maintain medical records and educate patients and their families about various health conditions. Notify physicians of significant changes in status such as medical emergency, change in medical status regarding response to medications, and related challenges. Complete certification/recertification orders and discharge summaries. Determine and provide the amount and type of nursing needed for assessed needs to each individual patient. Refer to licensed professionals to those patients requiring specialized care like speech therapy, rehabilitation services, etc.  Conduct patient care conferences on patients assigned to his/her care. Complete and submit OASIS assessments, reassessments, transfers, resumptions of care, discharges, and significant change in condition in accordance with Electra Home Health’s defined timeframes.  Participate in peer review and performance improvement as assigned. Participate in utilization review of medical records as assigned. Maintain HIPAA required and all other confidentiality while working with confidential matters on a daily basis. Appropriately utilize ICD-9/ICD-10 codes.  OTHER SIGNIFICANT REQUIREMENTS: Work well with other employees. Maintain confidentiality on a daily basis. Perform other functions and tasks as assigned. Participate in department on-call schedule. Follow standard precautions when providing direct patient care or handling potentially infectious materials. WHY EHD? QualificationsEDUCATION: Graduate of an accredited school of professional nursing required.   EXPERIENCE: Two years’ experience of nursing required. PERSONAL JOB-RELATED SKILLS: Knowledge of home health nursing care. Effective oral and written communication skills. Familiar with operating computers and ability to utilize computer systems. LICENSURE, REGISTRY, CERTIFICATIONS: Registered Nurse currently licensed by the state of Texas. Current CPR certification. Valid state driver’s license. PHYSICAL AND MENTAL REQUIREMENTS: Travel is required and reimbursed at the prevailing IRS business mileage rate. Duties of the position require extended periods of walking, sitting, and standing. Bending, stooping, and reaching is required with the lifting/carrying of items on occasion up to 52 pounds. The ability to comprehend and follow written and verbal instructions.
