Responsibilities:

Provide skilled nursing care to hospice patients in accordance with the plan of care.
Assess patients’ physical, emotional, and social needs.
Monitor patient conditions and make necessary adjustments to care plans.
Provide education and emotional support to patients and their families.
Collaborate with interdisciplinary teams to deliver holistic care.

Schedule

M – F
Day Shift: 8a – 5p
Every 7th weekend

Qualifications

Graduate of an accredited nursing program.
Current RN licensure.
Valid Driver’s License
