What You’ll Do
- Conduct medical assessments and support intake/placement decisionsÂ
- Develop and oversee individualized care plans in collaboration with physicians and interdisciplinary teamsÂ
- Monitor and update medical records, physician orders, and medication administration recordsÂ
- Administer medications and ensure compliance with established protocolsÂ
- Coordinate care with primary care providers and specialists; attend appointments as neededÂ
- Support individuals with complex or high-risk medical conditionsÂ
- Provide hands-on training to staff, caregivers, and families on medical care and equipmentÂ
- Promote overall health, wellness, and preventative care strategiesÂ
- Participate in care team meetings (IHP/POC) and contribute to ongoing health planningÂ
- Provide guidance and support to staff regarding home care and medical equipmentÂ
- Participate in an on-call rotation as neededÂ

What You’ll Bring
Qualifications:
- Graduate of an accredited RN programÂ
- Active RN license in the state of practiceÂ
- At least 1 year of experience working with medically complex individualsÂ
Â
Certifications:
- Current CPR/First Aid certificationÂ
- Valid driver’s license, auto insurance, and reliable transportation (if transporting individuals)
