What You’ll Do

- Conduct medical assessments and support intake/placement decisions 

- Develop and oversee individualized care plans in collaboration with physicians and interdisciplinary teams 

- Monitor and update medical records, physician orders, and medication administration records 

- Administer medications and ensure compliance with established protocols 

- Coordinate care with primary care providers and specialists; attend appointments as needed 

- Support individuals with complex or high-risk medical conditions 

- Provide hands-on training to staff, caregivers, and families on medical care and equipment 

- Promote overall health, wellness, and preventative care strategies 

- Participate in care team meetings (IHP/POC) and contribute to ongoing health planning 

- Provide guidance and support to staff regarding home care and medical equipment 

- Participate in an on-call rotation as needed 

 

What You’ll Bring

Qualifications:

- Graduate of an accredited RN program 

- Active RN license in the state of practice 

- At least 1 year of experience working with medically complex individuals 

 

Certifications:

- Current CPR/First Aid certification 

- Valid driver’s license, auto insurance, and reliable transportation (if transporting individuals)

Additional Requirements
