Job Summary

The Clinic Registered Nurse (RN) provides direct patient care through nurse-led visits for acute conditions, and preventive health services; following established clinical protocols. This role includes clinical assessment, documentation, and appropriate billing of services. The RN supports clinic operations by assisting providers with medical care, performing nursing procedures, performing triage to determine the appropriate level of care, and communicating diagnostic and laboratory testing. The position contributes to the overall efficiency and quality of care delivered within the care team setting. Within scope of job, requires critical thinking skills, decisive judgement and the ability to work with minimal supervision. Must be able to work in a fast-paced, multidisciplinary team environment.

Job Duties and Responsibilities

  • Conduct nurse-led visits for acute and wellness needs in alignment with established clinical protocols and standing orders.
  • Perform comprehensive assessments, identify patient care needs, and initiate appropriate interventions and follow-up care.
  • Document and manage care and services provided clearly and accurately in the electronic medical record (EMR).
  • Provide provider support through in-basket urgent needs, real-time patient assistance, and comprehensive clinical care, including treatments, procedures, and maintaining communication among the care team in accordance to protocols and/or regulatory requirements.
  • Educate and engage patients in self-care practices and health promotion strategies.
  • Referral management, care transitions, and patient follow-up as needed to support continuity of care.
  • Participate in quality improvement activities, team huddles, and staff meetings to support clinic goals and enhance patient outcomes.
  • Respond to rapid response as scheduled.
  • Special projects and other related duties as assigned.

Required Education and Experience

  • Graduate of an accredited school of nursing
  • Minimum of 2 years nursing experience in a clinical setting- required
  • BSN preferred.

Required Licenses, Certifications and/or Registrations

  • Current and unrestricted RN license.
  • Current BLS certification required.

Required Knowledge, Skills, Abilities

  • Working knowledge of community/public health, patient care coordination and case management.
  • Ability to cultivate effective partnerships and work in collaboration with providers, other health care professionals, caregivers, and patients. Strong leadership, facilitation, delegation, and coaching skills.
  • Proficient in the use of current technology, including Microsoft Office products.
  • Ability to develop processes and systems and to maintain accurate documentation and records.
  • Professional and effective written and verbal communication skills. Ability to identify and employ communication strategies appropriate to the audience.
  • Demonstrated leadership, communication, education, collaboration and counseling skills.
  • Ability to work independently performing a wide variety of assignments that require the use of independent judgment, problem-solving, organization, and prioritization skills.

Work Shift:

RN Contract 8 hour shifts in a 40 hour work week
