Per-Diem Pharmacist – Community Health Net (Erie, PA)

Position Overview

Community Health Net is seeking a licensed Per-Diem Pharmacist to support our outpatient pharmacy services in Erie, Pennsylvania. The Pharmacist is responsible for the safe dispensing of medications, patient counseling, and collaboration with an interdisciplinary healthcare team to ensure high-quality, patient-centered care.

This role is ideal for pharmacists seeking flexible scheduling, meaningful community impact, and experience in a community health / ambulatory care setting, including service to underserved populations.

Key Responsibilities

  • Dispense prescription medications accurately in compliance with prescriber orders, Pennsylvania pharmacy laws, and federal regulations

  • Compound medications according to professional standards and organizational policy

  • Provide patient-centered medication counseling on dosage, administration, side effects, interactions, and storage

  • Verify patient understanding of medication therapy and prescriber instructions

  • Monitor medication therapy for safety, effectiveness, and adherence

  • Maintain compliance with controlled substance regulations (DEA, PA Board of Pharmacy)

  • Collaborate with physicians, nurses, behavioral health providers, and care teams to support coordinated patient care

  • Maintain accurate pharmacy documentation and records within the EHR and pharmacy systems

  • Deliver professional, compassionate service to patients, staff, and community partners

  • Support daily pharmacy operations and perform related duties as assigned

Required Qualifications

  • Active Pennsylvania Pharmacist License (required)

  • Active Controlled Substance Registration (DEA) (required)

  • Strong knowledge of:

    • Prescription dispensing and verification

    • Dosage calculations and pharmaceutical compounding

    • Pharmaceutics, medications, narcotics, and poisons

    • Pennsylvania and federal pharmacy laws and regulations

  • Strong clinical judgment and attention to detail

  • Excellent verbal and written communication skills

  • Ability to work independently and as part of a multidisciplinary healthcare team

Preferred Qualifications

  • Retail pharmacy, outpatient pharmacy, or clinical pharmacy experience

  • Experience in community health centers, FQHCs, or underserved populations

  • Familiarity with EHR and pharmacy management systems
