Home Health Physical Therapist (PT) - North County Inland/Coastal

JTC Home Health Inc. is seeking a skilled and compassionate Per Diem Physical Therapist to provide exceptional care to our homebound patients in the Oceanside and Escondido areas, including nearby North County communities. The North County Role

As a vital member of our interdisciplinary team, you will travel to patient homes in the Oceanside, Escondido, Vista, San Marcos, and Carlsbad regions to deliver personalized physical therapy services.

Setting: Home Health
Territory: Oceanside, Escondido, and surrounding North San Diego County.
Schedule: Per Diem/PRN - Choose your own hours based on patient needs and your availability.
Compensation: Highly competitive per-visit rate.

Key Responsibilities

Perform comprehensive physical therapy evaluations and develop individualized Plans of Care (POCs) to restore function, reduce pain, and prevent disability.
Provide skilled treatment including therapeutic exercise, transfer training, gait training, and patient/caregiver education.
Coordinate care effectively with nurses, occupational therapists, speech-language pathologists, and physicians to ensure holistic patient recovery.
Ensure timely, accurate, and compliant clinical documentation using an Electronic Medical Record (EMR) system.
Monitor patient progress, communicate changes in condition to the physician, and participate in discharge planning.

Qualifications

Current and unrestricted Physical Therapist (PT) license in the State of California.
Minimum of 1 year of clinical experience as a PT (Home Health experience is strongly preferred).
Current CPR certification.
Doctor of Physical Therapy (DPT) degree is highly desirable.
Valid driver's license, reliable transportation, and proof of auto insurance.
Strong organizational skills and an independent work ethic.
