The Role: What You'll Be Doing (The Impact)

As a core member of the Stafford Services clinical team, you will be the front-line leader driving optimal outcomes and quality improvement.

• Clinical Autonomy: Provide expert routine and acute medical care directly to SNF residents.
• Comprehensive Management: Conduct thorough assessments, manage complex chronic conditions, and oversee personalized care plans.
• Collaborative Leadership: Partner seamlessly with facility nursing leadership and attending physicians to ensure integrated care.
• Quality Champion: Actively support quality improvement initiatives, focusing specifically on reducing preventable hospital readmissions

Your Toolkit: Qualifications

• Active and unrestricted Nurse Practitioner License in the State of Ohio.
• Active Nurse Practitioner Certification.
• Experience Bonus: Skilled Nursing/Rehab experience is preferred.
• New Grad Welcome: Will consider highly motivated new graduates, though 3+ years of prior RN experience is preferred for this specialized setting.
