Support Service Worker

Program: Supported Living/Ongoing Support

Status: Regular, Full Time, Part Time

FLSA Status: Nonexempt

Supervisor: Program Director, Program Manager

Classification Type: Full-time, part-time, or on call; schedule based on program needs

Location: Community-based service locations

Hourly Rate: $16.90 - $26.23 per hour


Position Summary

The Support Worker provides monitoring, support, training, and encouragement to individuals with disabilities who live in their own homes or are preparing to transition into their own homes. This position supports individual goals, daily living needs, community access, health and safety, and independence while maintaining respectful, person-centered services in accordance with HCAR policies and program expectations.


Essential Duties and Responsibilities

Essential duties and responsibilities include the following. Other duties may be assigned based on program and organizational needs.

路 Implement objectives identified in consumers' Individual Support Plans and provide support consistent with assigned service needs.

路 Provide monitoring, encouragement, and skill-building support related to daily living, independent living, social participation, housing stability, and community access.

路 Support consumers with emotional support, nutrition awareness, house finding, budgeting, shopping, household management, and money management skills.

路 Assist consumers in accessing community resources, appointments, services, and other supports as appropriate.

路 Provide transportation to appointments, community activities, errands, and assigned service locations when required and approved.

路 Maintain accurate and timely daily consumer support notes, service documentation, and progress information.

路 Communicate with the Program Director, case managers, Regional Center representatives, service providers, family members, and other agencies as appropriate.

路 Attend staff meetings, program meetings, and required training as assigned.

路 Assist consumers with self-help and personal care activities, including feeding, toileting, dressing, hygiene, and other support needs assigned and trained.

路 Assist consumers with special devices, including wheelchairs, walkers, prostheses, braces, or other adaptive equipment as needed.

路 Lift, transfer, or assist adult consumers as required using proper training, safety procedures, and assigned support plans.

路 Submit semi-annual progress reports and other required documentation related to consumer services and support needs.

路 Identify and report health, safety, behavioral, environmental, or service concerns to the appropriate supervisor in a timely manner.

路 Maintain confidentiality of consumer information and follow HCAR policies, safety practices, mandated reporting requirements, and applicable program procedures.

路 Perform other job-related duties as assigned to support consumers, program operations, and organizational needs.


Supervisory Responsibilities

This position does not provide direct supervision of employees.

Minimum Requirements

路 High school diploma or GED required.

路 Minimum of six months of related experience working with individuals with disabilities is preferred.

路 College coursework in social welfare, sociology, psychology, education, social services, or a related field is desirable.

路 Knowledge of community resources and the ability to support consumers in accessing appropriate services and support.

路 Ability to read and understand safety rules, service plans, documentation requirements, operating instructions, and organizational procedures.

路 Ability to complete routine reports, progress notes, correspondence, and required service documentation accurately and on time.

路 Ability to communicate clearly and professionally with consumers, support teams, HCAR staff, Regional Center representatives, other agencies, and community members.

路 Ability to use basic math for time, rate, ratio, percentage, service tracking, and documentation needs.

路 Ability to apply practical judgment, solve routine problems, follow written and verbal instructions, and seek supervisory guidance when needed.

路 Ability to prioritize job responsibilities, manage time effectively, and maintain positive and professional relationships with consumers and support teams.

路 Ability to maintain confidentiality and handle sensitive consumer, medical, family, and service information appropriately.

路 Valid First Aid and CPR certification or ability to obtain within required timeline.

路 Ability to successfully complete required background clearance, fingerprinting, TB clearance, physical examination, and other employment-related screenings.

路 Valid California driver license, reliable transportation, proof of automobile insurance, and acceptable driving record required when driving is part of assigned duties.


Physical Demands

The physical demands described here are representative of those required to perform the essential functions of this position. The employee may frequently stand, walk, sit, stoop, kneel, use hands and fingers, reach with hands and arms, talk, and hear. The employee may occasionally crouch, crawl, taste, or smell depending on service needs and work setting. The employee may be required to lift and transfer adult consumers and lift or move objects up to 50 pounds using proper training and safety procedures. Specific vision abilities may include close vision, distance vision, peripheral vision, depth perception, and the ability to adjust focus. Work Environment

The work environment may include consumer homes, community settings, vehicles, agency locations, and other assigned service environments. The employee may occasionally work in outside weather conditions. The noise level is usually moderate but may vary depending on consumer support needs, household setting, and community location.
