In this role, you will help individuals access the care and support they need by coordinating referrals, facilitating transitions of care, connecting them with community resources, and reducing barriers to treatment. Coordinate referrals from internal and external referral sources, including hospitals, correctional facilities, state hospitals, healthcare providers, community partners, and other organizations seeking to connect individuals to CSS services, ensuring timely engagement and appropriate program placement. Support organizational access standards, including CCBHC 7-day access requirements. Assist individuals with navigating Medicaid, insurance enrollment, Medicaid reinstatement, presumptive eligibility, and other insurance-related needs. Coordinate transitions of care for individuals discharging from jail, inpatient psychiatric facilities, state hospitals, residential settings, and other levels of care. Assist individuals in overcoming barriers to treatment, including transportation, identification, documentation, benefits, and community resource needs. Conduct outreach and follow-up activities to support engagement and successful connection to services. Facilitate communication and collaboration between programs, providers, referral sources, community partners, and natural supports. Document care coordination activities, resource linkage, referrals, and follow-up efforts in accordance with agency standards and CCBHC requirements. Monitor referral outcomes, access metrics, and barriers that may delay connection to services. Maintain working knowledge of community resources, entitlement programs, insurance benefits, referral processes, and eligibility requirements. Utilize trauma-informed care, motivational interviewing, harm reduction principles, and person-centered approaches when engaging individuals. Assist consumers with access to CCBHC services regardless of area of residence. Support clients with coordination of healthcare, behavioral health, social service, housing, employment, and benefit-related resources. Participate in interdisciplinary staffing, transition planning meetings, and care coordination activities as assigned. Assist with data tracking, reporting, and monitoring activities related to care coordination, access to care, and continuity of services. Obtain and maintain Insurance Navigator certification and provide insurance navigation support as assigned QualificationsWHAT'S REQUIRED FOR THIS POSITION? Bachelor’s degree in Social Work, Psychology, Sociology, Criminal Justice or other related degree required. Must possess an active, valid driver’s license. Must obtain and maintain Insurance Navigator certification for both the State of Indiana and the Federal Government within six (6) months of hire.
