What does a Hospice Social Worker do?
Assess the psychosocial status of patients related to patient’s illness and progression
Develop and provide multiple community and governmental resources to support a patient in their home environment
Provides assessment in patient’s residence, carries out social evaluations, and plans interventions
Participate in the development of the individualized plan of care
Assist physician in understanding significant social and emotional factors
Participate in quality assessment performance improvement team
Serve as liaison between patients and community agencies
Maintain and develop contracts with public and private agencies
Assist with bereavement coordination
Qualifications:
A graduate of an accredited bachelor's or master's program in social work
Minimum of one year’s experience in health care is required
Experience with home health or hospice is strongly preferred
Possesses and maintains current BLS Certification
A licensed driver with an insured automobile
Requires weekend rotation: Saturday and Sunday 8-5 or 9-6.
A Tuesday through Saturday schedule is highly preferred but not required.
