Hospice RN Case Manager – Key Responsibilities
Provide skilled nursing care and symptom management to terminally ill patients.
Develop and manage individualized care plans in coordination with the hospice team.
Educate and support patients and families through end-of-life care.
Administer medications and treatments for comfort and quality of life.
Coordinate care with physicians, social workers, aides, and other team members.
Respond to urgent needs and provide crisis support when necessary.
Maintain accurate and timely documentation in the EMR.
Manage a caseload, scheduling and prioritizing visits based on patient needs.
Participate in interdisciplinary team meetings and plan-of-care updates.
Ensure compliance with hospice regulations and standards.
Qualifications:
Active Washington State Registered Nurse (RN) license.
Prior Home Health and/or Hospice experience is strongly preferred.
Current BLS certification from the American Heart Association.
Valid driver's license and insured, reliable vehicle.
