What You'll Be Doing

You'll work with a geriatric population in a skilled nursing and transitional care setting, helping patients regain the skills needed for daily independence.

Typical responsibilities include:

• Evaluating patients and developing individualized treatment plans
• Providing ADL retraining and functional task practice
• Teaching adaptive techniques and use of assistive equipment
• Transfer training and mobility-related safety strategies
• Fall prevention and environmental safety education
• Treating medically complex patients recovering from illness or hospitalization
• Supervising and collaborating with a COTA as needed
• Participating in interdisciplinary care planning with PT, speech therapy, nursing, and facility staff
• Supporting discharge planning and safe return home when appropriate

The focus is always on helping patients regain independence and confidence in daily life.

Qualifications
