Join a growing Home Health team that is dedicated to helping patients regain strength, mobility, and independence in the comfort of their own homes. We're seeking a compassionate, motivated Physical Therapist (PT) who enjoys working one-on-one with patients and making a lasting impact every day.

Position Responsibilities:

Perform comprehensive physical therapy evaluations and develop individualized treatment plans
Provide skilled physical therapy services in patients' homes
Help patients improve strength, balance, mobility, endurance, and overall function
Educate patients and caregivers on home exercise programs, fall prevention, and safe mobility techniques
Monitor patient progress and modify treatment plans as needed
Complete accurate and timely documentation
Collaborate with physicians, nurses, and the interdisciplinary Home Health team to provide exceptional patient care

Qualifications:

Current Indiana Physical Therapist (PT) license
Home Health experience preferred, but not required
Strong clinical assessment and communication skills
Compassionate, dependable, and patient-focused
Valid driver's license and reliable transportation
