Position Summary: The Clinical Case Manager is responsible for assisting the Clinical Director in coordinating treatment services including initial and ongoing coordination of care, advocacy, behavior intervention, aftercare, follow-up, and discharge planning. Case Management will work directly under the Clinical Director to ensure all policies and procedures regarding client care, discharge planning, and case management service needs.

Essential Functions

  • Conduct comprehensive assessments to identify clients' needs and develop individualized care plans.
  • Coordinate and advocate for the delivery of a range of services to meet clients' substance abuse treatment goals.
  • Collaborate with Clinical Director, healthcare team and external agencies to ensure continuity of care.
  • Provide support and guidance to clients in accessing community resources and support services.
  • Document all notes into EMR accurately and complete as it pertains to interaction with patients and the communication of their discharge plan and aftercare schedule.
  • Facilitate communication and collaboration among the treatment team to ensure a cohesive and person-centered approach.
  • Monitor and evaluate clients' progress toward treatment goals to assist with aftercare planning.
  • Participate in team meetings to discuss client care and treatment plans.
  • Provide crisis intervention and support as needed.
  • Performs other duties as assigned by the supervisor, which are consistent with the position and in compliance with agency policies and procedures.
