SSVF Healthcare Navigator



Location: Fairbanks Rescue Mission – Fairbanks, Alaska
Department: Supportive Services for Veteran Families (SSVF)
Reports To: SSVF Program Manager
Employment Type: Full-Time, Non-Exempt


Starting Pay: $25.00 per hour, depending on experience


Position Summary

The Fairbanks Rescue Mission is seeking a compassionate and detail-oriented SSVF Healthcare Navigator to assist Veterans and Veteran families participating in the Supportive Services for Veteran Families (SSVF) Program. The Healthcare Navigator plays a critical role in connecting Veterans experiencing homelessness or housing instability with medical, behavioral health, and supportive care services.

 

Working collaboratively with SSVF Case Managers and community partners, the Healthcare Navigator helps ensure Veterans have access to VA healthcare, community medical services, and mental health support, promoting improved health outcomes and long-term housing stability.

 

Key Responsibilities

Client Support & Care Navigation

  • Meet regularly with program participants to assess medical and mental health needs.
  • Develop individualized healthcare support plans for eligible Veterans.
  • Assist Veterans in accessing healthcare services including VA medical care, behavioral health services, and specialty providers.
  • Help coordinate and schedule medical and mental health appointments.
  • Provide reminders and support for upcoming healthcare appointments.
  • Assist with arranging transportation to and from medical appointments when necessary.

Service Coordination

  • Work closely with SSVF Case Managers to ensure Veterans receive comprehensive and coordinated services.
  • Provide or broker services to address participants’ healthcare needs, including:
    • VA benefits assistance
    • Medical and mental health referrals
    • Healthcare appointment coordination
    • Follow-up care and service linkage
  • Coordinate services with community healthcare providers, hospitals, behavioral health providers, and Veteran service organizations.

Documentation & Program Reporting

  • Maintain accurate and timely documentation of all client contacts and services provided.
  • Document case notes including date, type of contact, service provided, and outcomes.
  • Maintain organized participant records in accordance with SSVF program requirements and confidentiality standards.
  • Provide regular updates and reports to the SSVF Program Manager.

Client Transition & Aftercare

  • Participate in discharge planning and coordinate ongoing healthcare support when participants exit the program.
  • Ensure continuity of care through referrals and connection to long-term healthcare providers.

 

Qualifications

  • Associate’s Degree in Human Services, Behavioral Health, Social Work, or related field preferred, or equivalent professional experience.
  • Minimum two (2) years of experience in case management, healthcare navigation, or social services preferred.
  • Experience working with Veterans, homeless populations, or vulnerable populations strongly preferred.
  • Strong verbal, written, and interpersonal communication skills.
  • Ability to work effectively with individuals from diverse educational, cultural, and socioeconomic backgrounds.
  • Strong organizational and documentation skills.
  • Proficiency in basic computer systems, Microsoft Office, and email communications.
  • Ability to obtain military base access if required.
  • Valid Alaska Driver’s License required.

 

Work Environment

  • Office-based position at the Fairbanks Rescue Mission.
  • Standard weekday business hours with occasional flexibility to meet program needs.
  • Regular interaction with Veterans, healthcare providers, and community partners.
