Lead Primary Care Physician / Associate Medical Director

We are partnered with a mission-driven community health organization seeking a Board-Certified Primary Care Physician to join their team in a leadership-track role as an Associate Medical Director, with the opportunity to transition into Medical Director. This position is for physicians who are passionate about caring for complex patient populations and who bring strong clinical judgment, professionalism, and reliability.

This role is 100 percent onsite Monday through Friday 8am to 5pm, providing full-scope primary care while supervising and developing providers within a collaborative team environment.

Job Description

The Associate Medical Director delivers comprehensive primary care and provides leadership to physicians, APPs, nurses, behavioral health staff, and medical assistants. This individual will treat medically and socially complex patients, including those with chronic disease, substance use disorders, infectious disease, and behavioral health challenges.

Key Responsibilities

Clinical Care


• Provide comprehensive primary care to a managed patient panel
• Diagnose and manage chronic and acute conditions
• Conduct transitional and follow-up care for high-risk individuals
• Coordinate care with interdisciplinary support staff
• Comfortable prescribing opioids when medically appropriate
• Work with populations experiencing high medical and social complexity


Leadership & Oversight


• Coach and supervise clinical providers to elevate quality and best practices
• Lead population health huddles and support quality/utilization target achievement
• Collaborate closely with the Medical Director in clinical decision-making
• Participate in policy development, workflows, and value-based care initiatives
• Represent the center in clinical discussions and organizational planning

Requirements

Must Have


• Current Board Certification in Family Medicine or Internal Medicine (no exceptions)
• Minimum 5 years clinical experience post-training
 – Experience from FQHC or community health settings may allow some flexibility
• Recent hands-on primary care experience within the last 2 to 3 years
• Comfort prescribing opioids when appropriate
• Full-time onsite availability M-F, 8am-5pm
• Professional maturity and reliability consistent with leadership-track expectations


Acceptable with Conditions


• Infectious Disease or Addiction Medicine physicians may be considered if fully willing to practice primary care only, with no split percentage between specialties
• Preventive or Occupational Medicine backgrounds do not qualify alone
 – Must also have recent true primary care experience


Not Accepted


• New graduates or residents
• Providers awaiting board results (must be currently certified)
