At NorthBay Health, we are seeking a Licensed Clinical Social Worker (LCSW) in Fairfield, CA to provide direct clinical social work service, counseling, and case management services to Cancer Center patients and their families, including psychosocial assessment, development of a plan of care, implementation, and documentation of emotional, psychological, social, financial, environmental, or follow-up referral needs. 

Responsibilities:

  • Provide direct clinical social work services, including counseling and case management for Cancer Center patients and their families
  • Conduct psychosocial assessments and develop individualized plans of care addressing emotional, psychological, social, financial, environmental, and referral needs
  • Administer ACOS-approved distress screening tools and assess patients at key points in the cancer continuum, including diagnosis, pre- and post-surgery, treatment initiation, mid-treatment, and discharge, as well as other high-risk periods
  • Provide individual, family, and group counseling, education, advocacy, and patient navigation across all phases of cancer care, including prevention, diagnosis, treatment, survivorship, palliative care, end-of-life care, and bereavement support
  • Serve as an integral member of the survivorship program team, collaborating with physicians, nurses, nutritionists, therapists, and other allied health professionals to develop and implement survivorship services
  • Ensure timely and appropriate referrals for patients screening positive for moderate to severe distress
  • Maintain and update a comprehensive resource directory of behavioral health providers, pastoral care, palliative care, and other supportive services
  • Evaluate, document, and report distress screening outcomes in alignment with accreditation requirements and best practices
  • Document all assessments, screenings, and interventions accurately in the electronic health record and conduct quality improvement audits
  • Participate in interdisciplinary team meetings, tumor boards, and provide reporting to the cancer committee on psychosocial program initiatives
  • Monitor compliance with certification standards (including ACOS and other regulatory bodies) and support ongoing program development and quality improvement initiatives
  • Ensure all activities align with NorthBay Health’s mission, values, policies, procedures, and regulatory requirements
  • Perform other duties as assigned
