Certified Nursing Assistant Job Description

Reports to: Nurse in charge of their assigned residents.

Summary:  Employee provides direct care to nursing home residents, primarily for their activities of daily living, but also to include some medical care.

Equipment Used:  Blood Pressure Gauge, Thermometer, Stethoscope, Lifts, Whirlpool Tub, etc.

Environment:  Inside:  100%    Outside:  0%

Essential Duties and Responsibilities include the following:  Other duties may be assigned.

  • Assist residents in and out of bed, into wheelchair or chair with the correct use of a transfer (gait) belt.  May require assistance from another assistant or nurse depending on the weight and physical abilities of the resident.  Lift may be used – see policy in employee handbook.
  • Assist residents in dressing and undressing.
  • Assist resident in bathing. Including shampoos and nail care.
  • Assist resident in using bathroom and peri-care. Clean bedpans and urinals as required and needed.  Empty Foley catheter bags every shift and prn. Record output.
  • Assist residents with am and pm care.  (grooming, mouth care, etc.)
  • Take Vital Signs of residents as required and provide written/electronic record (T, P, R, B/P).
  • Assist residents to dining areas for meals.  Assist residents or feed residents who are unable to perform the task. Document meals and nourishment intakes.
  • Change residents bed linens and straighten resident’s rooms as required and prn.
  • Assist residents to turn or turn residents who stay in bed for long periods of time.  Residents should be turned and repositioned at least every 2 hours while in bed.
  • Required to document information regarding the resident’s health, Intake and Output, and report any changes in resident’s condition to the nurse in charge immediately – difficulty breathing, changes in skin integrity, bruising, swelling or feet or ankles, c/o pain, falls, behavior changes, s/s of illness, c/o verbal or physical abuse, and any other unusual occurrences.
  • Assist with admitting residents, discharging residents, or transferring residents.
  • Answer call bells promptly and make sure call bell is always within reach.
  • Report to nurse in charge each time before leaving unit and at end of shift.
  • Assist Rehab Aides, Bath Aides, Transportation Aide, and Therapy as requested or needed.  Get resident ready for appointments, therapy, or rehab, etc.
  • Must be able to consistently work cooperatively with residents, charge nurses, CNA’s and other staff members.
  • Must wear clean, neat, and modest uniform.
  • Must be punctual and dependable.
  • Must be accept duties politely and perform them with minimal supervision.
  • Must be able to read and understand Dietary Meal Cards to ensure meal trays are delivered to proper resident with appropriate items.  (ex: no sugar on diabetic trays, altered diets, thickened liquids, etc.)
  • Must be able to take resident's outside to smoke at assigned times, as requested.
  • Adhere to Flesher’s policies and procedures, employee handbook, resident’s rights, OSHA, State and Federal safety standard precautions, i.e. infection control, PPE’s, and isolation rooms.

Qualifications


Qualifications:  To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.  Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

Education:  State Certification is required and up to one-month related experience or training preferred.

Language Skills:  Be able to read and write and understand the English language.  Be able to read, write and comprehend simple instructions, short correspondence, and memos.  Ability to effectively present information in one-on-one to “resident or resident’s families” and other employees of the organization.
