RN Home Health Patient Care Manager

Full-time/Direct-Hire Position | Monday – Friday | 8 a.m. to 5 p.m.

The RN Home Health Care Manager is integral to delivering compassionate, high-quality home health services.

Through effective coordination, clear communication, and adherence to professional and regulatory standards, this leader drives a culture of excellence, safety, and trust — shaping quality outcomes and making a meaningful difference in the lives of patients and the professionals who serve them.

Position Responsibilities:

  • Provide clinical supervision to ensure patient care meets professional standards, agency policies, and regulatory requirements
  • Coordinate and monitor patient care delivery
  • Supervise clinical staff to ensure consistent, appropriate service
  • Maintain clear communication with patients, caregivers, referral sources, and field and office staff
  • Drive quality outcomes by meeting department goals and supporting improvement initiatives
  • Uphold compliance through required training, accurate reporting, and cooperation with audits
  • Registered nurse with current licensure to practice nursing.
  • Previous experience in home care setting with two years management or supervisory experience, preferred.
  • Knowledge of accepted professional standards and practice, Medicare Conditions of Participation, and federal, state, and local regulatory requirements
