Home Care Case Manager $28.58/hour

Position Summary: 

This position is responsible for initial client assessments, reassessments, and ongoing case management of home care clients in compliance with Elder Affairs and agency guidelines.

Principal Accountabilities:

  1. Assures that clients are assessed and reassessed on a timely basis:
    1. Conducts a comprehensive evaluation to determine the full scope of clients’ needs.
    2. Determines eligibility for programs.
    3. Develops appropriate service plans that meet consumer choice and budget guidelines.
    4. Partners with vendors to implement home care services.
    5. Monitors clients for changes requiring adjustments to service levels.
  2. Manages cases on an ongoing basis:
    1. Assists clients with problem solving and makes referrals as needed.
    2. Communicates with contracts administrator regarding vendor quality issues.
  3. Determines eligibility for public benefits:
    1. Assists with applications for public benefits and health insurance and problem resolution related to benefits.
    2. Coordinates safe hospital and nursing facility discharges.
    3. Collaborates with other programs and agencies.
    4. Conducts skills training for consumers enrolled in the consumer directed care program.
  4.  Documents phone calls, visits, and all activity in accordance with Elder Affairs’ and agency requirements; maintains client records accurately and within the mandated timeframe. 
  5. Participates in office coverage responsibilities on a rotating basis. .

Qualifications: 

Education: Associates degree degree in human services or related discipline required; Bachelors degree preferred.

Experience: Elder service helpful. 

Special Skills/Knowledge: 

Excellent oral and written communications skills, 

ability to effectively relate to a wide variety of personalities, 

strong assessment skills,

 basic computer skills, 

organizational and time management skills, 

ability to multi-task and work in a fast-paced, 

high volume work environment.

 Driver’s license and reliable transportation required.
