 JOB DESCRIPTION

TITLE: Caseworker - DCF

JOB CLASSIFICATION: Full Time – Non-Exempt

REPORTS TO: Assigned Supervisor

JOB SUMMARY:

Under the supervision of their assigned supervisor, the Caseworker is responsible for ensuring that all clients receive professional, safe care in a well maintained and secure environment; limit setting, behavior management, family work, the teaching of daily living skills, facilitation of medical/dental appointment and follow-up, school tracking, and facilitate psycho-educational groups. The Caseworker acts as a liaison with the client’s family, Department of Children & Families (DCF) Social Worker, school placements, and medical/dental practices to ensure that families and providers understand clients’ needs and that basic needs are being met.

JOB FUNCTION:

Program/Department Responsibility:

ï‚· Advocate for clients in their home, school, and the community according to the individual treatment plan on a daily basis.

ï‚· Identify community providers and medical and dental needs of clients following intake.

ï‚· Schedule appointments as needed with medical/dental providers.

ï‚· Seek out medication orders/PRN protocols forms/documentation of immunizations and most recent routine appointments for each client.

ï‚· Facilitate medical/dental/eye appointments for clients as needed to best maintain optimal physical/medical/dental health of all clients

ï‚· Share information with clinical team, administration, nurse and supervisors following appointments/phone consults through thorough and efficient documentation.

ï‚· Complete daily school tracking requirements

ï‚· Participate in weekly client review with administrative team

ï‚· Coordinate urine toxicology screenings for youth when ordered by provider

ï‚· Present in a professional manner and attire while representing the OCY DCF Program including personal vehicle, which will be used in fulfilling job responsibilities.

ï‚· Complete all paperwork and documentation as needed including but not limited to: Medical Encounter Forms, Med Order Forms, PRN Protocol Forms, Narratives, Electronic Appointment Spreadsheet. School tracking log, Youth Readiness Assessments, etc.

ï‚· Develop professional relationships with local health care providers as well as employers in order to facilitate collaboration between DCF program and local community.

 Work with DCF and parents to empower parents to be active in all aspects of their child’s health and education through open communication and collaboration with parent/guardian when appropriate.

ï‚· Provide rotating on-call clinical coverage and be available for after-hours emergency situation or intake.

 

 

ï‚· Participate in all EEC and CARF related requirements, meetings and visits.

ï‚· Contribute to a positive work environment by illustrating pride in job performance, enthusiasm and cooperation with co-workers.

ï‚· Facilitate and support PAYA programming for youth.

 

Supervision of Personnel:

N/A

Fiscal Responsibility:

ï‚· Adhere to all fiscal policies and procedures such as petty cash, purchase orders and vouchers.

Facility/House Maintenance Responsibility:

ï‚· Maintain upkeep and cleanliness and ensure security of the OCY assigned site

ï‚· Maintain daily upkeep, cleanliness and organization of office use and program equipment.

 

Essential Job Functions:

ï‚· Ability to communicate with clients, families, and collaterals in person and by phone

ï‚· Daily attendance and scheduling flexibility

ï‚· Ability to assist with the restraining of residents up to approximately 200 lbs.

ï‚· Visual and auditory acuity to maintain program security required

ï‚· Ability to stand, walk, crouch intermittently

ï‚· Must be able to push, pull and lift up to 25 lbs. on occasion

 Must be able to drive, provide a safe driving record and have a valid driver’s license

ï‚· Must be able to pass a drug screen

 

Interdivisional/Community/Volunteer Responsibility:

N/A

Education/Experience Required:

 Bachelor’s Degree candidate in a Human Services related field or with 1 – 5 years of applicable experience.

There will be occasions that the use of your personal vehicle will be expected and required. You will be reimbursed for the mileage completed during an OCY assignment.
