The Opportunity:

Silvergate Fallbrook, part of AmeriCare Health & Retirement, is seeking a compassionate and skilled Part-Time LVN to support our award‑winning senior living community. If you’re a nurse who values teamwork, resident-centered care, and a supportive work culture, we’d love to meet you.

What You’ll Do:

• Provide clinical oversight and clear direction to caregiving staff
• Assess residents, document changes, and communicate with physicians and families
• Perform diabetic care, medication tasks, and follow physician orders
• Coordinate medical appointments and support proactive health management
• Partner with Home Health/Hospice teams to ensure comfort and continuity of care
• Train, coach, and support caregiving staff
• Uphold infection control and regulatory standards (Title 22)

Qualifications

Minimum Qualifications

• Current California LVN license
• Current CPR and First Aid certification
• 2 years nursing experience (assisted living, memory care, or SNF preferred)
• Strong communication, leadership, and teamwork skills
• Knowledge of gerontology, dementia care, and resident rights
• Ability to remain calm and effective in emergencies
