Bilingual Medical Case Manager

Location: Keystone Infectious Diseases, Chambersburg
Status: Full-Time, Benefits Eligible

Schedule: Monday through Friday, 8:15 a.m. to 4:45 p.m. Candidates must be available to participate in a rotating Tuesday clinic schedule from 9:00 a.m. to 6:00 p.m.

Keystone Infectious Diseases is seeking a compassionate and dedicated Bilingual Medical Case Manager to join our team.

In this role, you will coordinate medical and supportive services for individuals living with HIV. The Case Manager works closely with patients to promote adherence to treatment plans prescribed by their HIV provider and to help overcome barriers to accessing and maintaining HIV primary care. Services are provided both in the office and throughout the community and may include face-to-face and non-face-to-face interactions.

EDUCATION OR EXPERIENCE:  Registered Nurse; and/or have a bachelor’s degree in social work, psychology, sociology or other related field; or bachelor’s degree in a non-similar field; and two years’ experience in case management, social work and/or a Community Health Worker Certification.

Ability to read, write, and speak Spanish fluently is required
