PROGRAM COORDINATOR - Full-Time, Fargo, ND

Job Responsibilities:

The position provides person-centered support and coordination for individuals with developmental disabilities. Responsibilities include:

  • Build positive, trusting relationships while exploring and supporting each person’s dreams, goals, and desires.
  • Develop, coordinate, implement, and monitor Person-Centered Support Plans (PCSPs), including assessments, meetings, documentation, and follow-up.
  • Coordinate with interdisciplinary team members, families, guardians, and other providers to ensure effective and consistent supports.
  • Maintain required documentation and records, including use of Therap and materials required by the Human Rights Committee and Behavior Management Committee.
  • Assist individuals in maintaining eligibility for financial and community benefits.
  • Provide crisis intervention, counseling, problem-solving, and referrals to additional resources when needed.
  • Participate in an on-call rotation and respond to situations as required.
  • Complete all required training and participate in additional assignments and organizational initiatives as needed.


Required Qualifications:

  • Bachelor’s degree in Social Work or a Human Services-related field.
  • At least one year of experience working with people with developmental disabilities.
  • Strong verbal and written communication skills and the ability to interact positively and effectively with a variety of individuals.
  • Demonstrated ability to build positive, professional relationships with people supported, families, and team members.
  • Driver’s license; ability to meet organizational vehicle and personal auto insurance requirements.

 

Schedule:

  • Flexible schedule as this position requires covering a variety of shifts.  Typically 8:00 am - 5:00 pm, but will need to work some evenings and weekends as a Direct Support Professional.
  • Participate in the on-call rotation and cover shifts as needed.
