What You'll Do
- Evaluate and treat patients throughout Southern and Southwest Minnesota in their homes.
- Design individualized treatment plans focused on restoring mobility and independence.
- Educate patients and caregivers to improve long-term outcomes.
- Collaborate with physicians, nurses, occupational therapists, speech therapists, and other members of the care team.
- Monitor progress and adjust treatment plans as patients improve.
- Complete timely and accurate documentation.
What We're Looking For
- Active Minnesota Physical Therapist (PT) license.
- Strong clinical reasoning and patient-centered approach.
- Excellent communication and organizational skills.
- Comfortable working independently across a regional territory.
- Valid driver's license and reliable transportation.
