Catholic Health Home Care is looking for passionate, dedicated LPNs to provide comprehensive care to patients at home.

Team 1 Covers: Bayville, Bethpage, Brookville, East Norwich, Glen Cove, Glen Head, Glenwood Landing, Greenvale, Hicksville, Jericho, Laurel Hollow, Levittown, Massapequa, Massapequa Park, Mill Neck, Muttontown, Oyster Bay, Plainedge, Plainview, Seacliff, Syosset, & Woodbury.

5x 8 hour shifts (8:30AM to 4:30PM) - Rotating Weekends

Job Details

Our Home Care LPN’s:

Case manage, assess, plan, develop and implement a comprehensive home health care plan for their patients.
Measure vital signs, Administer IVs, change dressings, clean wounds as needed
Provides skilled nursing care, preventative rehabilitative procedures, and prescribed treatments with a variety of patient populations within various potentially complex home situations.
Work closely with a supportive patient care team including physicians, therapy and social services, pastoral care, home health aides and others for your assigned caseload.
Provide patient and family education for disease management and health promotion s directed by physician’s plan of treatment.

Minimum Qualifications:
- New York State Licensed Practical Nurse (LPN) license
- Minimum of one year of recent hands on experience
- Basic Life Support Certification

Salary Range

USD $33.00 - USD $40.00 /Hr.
