Purpose of the PositionRainbow Community Care is currently accepting applications for a dedicated Full Time CNA to work at the Inpatient Center in Johnson Creek. This person will work 11am to 8pm four days per week. Days will alternate based on week. Key Responsibilities Assists with ambulation, transferring, and changing position of patients.  Assists the patient with personal grooming, toileting, incontinence care, bathing, skin care, nail care, dressing, transferring and walking.  Assists with preparing meals and assists patients with eating and drinking.  Gives emotional and psychological support to the patient/family. Maintains a clean, safe and comfortable environment. Performs light housekeeping duties as assigned. Reads, understands, and follows the plan of care.  Maintains up-to date patient records and documents cares provided to the patient. Reports changes in patient condition according to Rainbow policies.  Professional Qualifications Valid Wisconsin driver’s license and proof of current car insurance are required upon hire and must be maintained throughout employment. Must be able to drive to and from other office/meeting locations as necessary.  Current Wisconsin Nursing Assistant Certification is required.  Minimum of 6 months CNA experience preferred. Hospice experience preferred.   Personal Qualifications Must be able to maintain healthy boundaries and positive interpersonal relationships. Must be tactful and courteous in communication with others, whether in-person, by phone, or in writing. Must act with integrity and demonstrate good work ethics including reliable attendance and punctuality. Ability to be self-directed and handle stressful and emergency situations. Ability to maintain patient, family, and staff confidentiality.
