Role Profile- Registered Practical Nurse (RPN)
Mission:
Reporting to the Director of Care, the Registered Practical Nurse (RPN) is responsible for providing Resident centered and goal-oriented nursing care in accordance with standards of practice and provincial legislation. Working collaboratively within the interdisciplinary team, the RPN will participate in the development, implementation, monitoring and evaluation of care and clinical services to enhance the provision of Resident care.

Key Responsibilities:
•
Maintains accurate and current health records, clinical assessments and documentation, reflecting the Residents abilities, needs and preferences and in accordance with the College of Nurses standards and organizational policies and procedures.
•
Implements and evaluates the effectiveness of care and services as outlined in the plan of care, while making appropriate revisions to goals and interventions in collaboration with the interdisciplinary team as required.
•
Assist with scheduling and participating in Resident care conferences to ensure co- ordination of services where applicable.
•
Work with the Director of Care/designate to facilitate Resident move in, discharge and transfers to and from the home.
•
Perform necessary nursing functions to facilitate the delivery of safe Resident care, including clinical assessment, medication administration, care treatments and maintain documentation of care provided in compliance with regulatory requirements and organizational policies and procedures.
•
Process physician orders, ensure drugs and narcotics are secured and administered as ordered or discarded as required in compliance with all regulatory requirements, clinical scope and organizational policies and procedures.
•
Work with preferred pharmacy regarding Resident orders and to ensure appropriate inventory of emergency drugs where applicable.
•
Facilitate and provide resident/family education related to Resident conditions, care and services where required.
•
Plan, coordinate, manage & evaluate the Resident’s plan of care routinely and as required, working with the Resident/substitute decision maker, physicians, interdisciplinary team and contracted service providers; where applicable.

Coordinate Resident care assignments and ensure safe, efficient and timely delivery of care services.

Respond to and manage significant changes in residents’ condition and or medical emergencies; provide emergency first aid.
•
Maintain Resident safety and work in a manner that minimizes risk of adverse outcomes.
•
Supervise, train, delegate and direct unregulated care providers as required.
•
Respond to Resident/family concerns immediately and communicate to the Director of Care/Executive Director and other departments as applicable to ensure appropriate follow up and corrective action.
•
Assist the Director of Care in the coordination, implementation and monitoring of Infection Prevention and Control, Health and Safety and Clinical Programs and Quality/Safety Plans for the Nursing Department.
•
Work collaboratively with the interdisciplinary team in the development, implementation and sustainability of clinical and quality improvement measures and programs.
•
Work in a manner that maintains Resident rights, dignity and preserves confidentiality.
•
Uphold and promote core values and philosophy relating to ethics, morality, and integrity as set out in the Code of Conduct.
•
Complete all other duties as assigned.
•
Adheres to responsibilities and requirements for reporting adverse resident occurrences subject to regulatory and legislative direction.
Required Education, Credentials and Experience:
•
Current registration as a Registered Practical Nurse with the applicable provincial the College of Nurses.
•
Experience working in a senior’s environment and with unregulated care providers preferred.
•
Education and experience in gerontology, rehabilitation, community/complex/long term care an asset.
•
Must possess proficient written and oral communication, critical thinking, care coordination, time management and customer focus skills.
•
Overall competency working under pressure with a high stress tolerance.
•
Must have demonstrated sound clinical knowledge and competent skill set.
•
Must have demonstrated the ability for self-reflection and professional accountability for work and performance.
•
Must maintain competencies as per the governing College standards.
•
Computer proficiency and experience with Microsoft office applications.
•
Current (within 6 months) Vulnerable Position/Sector Screening (VPS).
