Patient Care Monitoring: Monitor patient care and ensure compliance with hospice regulations, company policies, and industry best practices. Care Coordination: Coordinate with the medical director, physicians, and other interdisciplinary team members to develop individualized care plans. Clinical Assessments: Conduct regular clinical assessments and audits to maintain the highest standards of patient care. Education and Training: Provide education and training to clinical staff to enhance their skills and knowledge in hospice care. Staffing and Scheduling: Assist with staffing and scheduling to ensure appropriate coverage and continuity of care. Resource for Staff and Families: Act as a resource for clinical staff and families, addressing concerns or questions regarding patient care. Documentation Management: Oversee and maintain documentation of patient care, ensuring accurate and timely recordkeeping in accordance with regulatory guidelines. Policy Development: Participate in the development of policies and procedures to improve hospice care practices and patient outcomes. Hospice Philosophy Promotion: Promote the hospice philosophy of care, ensuring a focus on dignity, comfort, and respect for each patient. What You’ll Bring: Registered Nurse (RN) with current licensure in Maine. At least 3-5 years of clinical experience in hospice, palliative care, or a related field. Previous leadership or management experience in a clinical setting preferred. Strong knowledge of hospice regulations, Medicare guidelines, and clinical best practices. Excellent communication and interpersonal skills, with a compassionate and patient-centered approach. Ability to work independently, as well as part of a multidisciplinary team. Strong organizational skills and the ability to handle multiple priorities in a fast-paced environment. BLS/CPR certification required; additional certifications (CHPN, etc.) preferred.
