Per diem remote social worker supporting all outpatient palliative 

Job Description
Provide complex social work services as a member of an interprofessional team; care that is informed by comprehensive biopsychosocial-spiritual assessment, intervention and treatment.  Demonstrates strong advocacy skills; Requires activated and engaged social worker exerting discretionary effort.  Values holistic patient-centered and family-focused care. Will work effectively on a complex interprofessional team. Maintains ethical principles and professional standards of practice in adherence to NASW Code of Ethics. The social worker is a part of the health care team and establishes and maintains a relationship with community agencies, has a working knowledge of community and financial resources, and assists with appropriate agency referrals and short and long-term planning as needed.
 



Licensed Clinical Social Worker (LCSW) required.
Master's degree from a social work program accredited by the Council on Social Work Education required.
One to two years LCSW work experience in a healthcare setting preferred.
