COMPENSATION AND BENEFITS Family Medicine (FM)

• Up to $300,000 with clinical experience
• Sign-on Bonus is $20,000, another $10,000 paid every 6 months 
• Medical, Dental, and Vision coverage + 403B Investment Program
• PTO days and 8 Holidays + 1 Floating Day
• CME: 5 days for $3,000

• Outpatient clinic only with bread and butter cases
• Monday to Friday with 4 hours a week for administrative time
• No weekends or after-hours coverage
• Student Loan Repayment: Allen County, Indiana has a HPSA score of 21 for Primary Care

• Board Eligible or Board Certified is accepted
• Will consider Visa Candidates at this time
• Open to late-career physicians
• Good bedside manners, warm personality, compassionate with patients
