Person Centered Plan Manager-CBDES


Position Summary: Collaborates with participants and their team to develop and implement individualized ISP goals aimed to increase employability, independent living, and community skills. The PCPM works collaboratively with the Program Director and Division Director for oversight of the staff and for overall support. Supervises assigned direct support staff members in addition to providing direct support to assigned individuals as needed. Oversee morning and afternoon transitions.


 Salary & Benefits:


$3500 Sign-On Bonus for New Hires! (paid in 2 installments at 3 months and 6 months with successful completion of Orientation)


$1500 Referral Bonus for Current Staff!


$25.95 Per Hour

Schedule: Monday-Friday 8am-4pm (40 hr/week)


 Responsibilities:


 Develops, implements and monitors Individual Service Plans (ISP) and clinical support strategies using Positive Behavior Supports (PBS) guidelines.


 • Serves as the primary contact for an assigned caseload. 


• Collaborates with the Program Director, Division Director, and Senior Leadership in the coordination and execution of agency and program goals. 


• Data entry into electronic systems and maintenance of electronic files. HCSIS, Outlook, Excel, Icentrix and other programs will be used daily within this position. 


• Provide input and assistance in determining intake, transfer, and discharge decisions regarding individuals by utilizing program services.


 • Supervises, supports, trains and mentors assigned direct support staff. 


 • Assists in planning and implementation of developmental skills training through group activities, therapist recommendations, and other strategies. 


• May provide transfers for individuals who use wheelchairs using safe lifts and transfer. 


• Enhances and promotes The Arc of Opportunity’s strategic initiatives, culture, and organizational mission and values through positive promotion and representation while actively engaging in professional networking. 


 • Maintains required certifications and re-certifications as required and in accordance with agency, State and Federal guidelines. 


• Driving of a motor vehicle and/or van, agency and/or personal


• Administer medication in accordance with MAP regulations


 • Other duties as assigned.


Qualifications: 


Valid driver’s license and acceptable driving record and CORI is required at all times during employment with The Arc of Opportunity. 


• High School Diploma or GED required. 


• Must have experience with case managing in a relevant health care setting.


 • Supervisory experience preferred but not required.


 • Ability to gather and apply factual information, exercise sound judgment, problem solve, and apply principles of influence systems such as motivation, team building, accountability, coaching/counseling of staff and leadership. •


 Cross-functional communication skills, in verbal and written English, which convey clear direction, desired results and vision to staff, individuals/families served, government agencies, community partners, funding sources, and senior management. 


 • Computer literacy skills 


• Ability to: o Lift up to 50 lbs. o Kneel, bend push, pull and carry items. o Physically assist individuals according to their needs and perform restraints if necessary. o Sustain periods of walking, standing and sitting.
