Position Summary:
The Medication Reconciliation Technician provides distributive pharmacy services, assistance and support for the registered pharmacist. Responsible for collecting accurate home medications lists for all inpatients.

How you’ll contribute

A Medication Reconciliation Pharmacy Technician who excels in this role will:

Obtain accurate home medication history via patient/caregiver interviews, contacting outside pharmacies, physicians’ offices, long term care facilities, or any other reasonable means
Obtain and update patient medication allergy information in the electronic medical record
Enter and maintain medication history in Epic
Review home medication list for duplications and/or omissions prior to entering into the patient’s electronic medical record.
Assist in the recording of statistics for medication reconciliation
Participate in committees or teams that are relevant to the practice area and medication reconciliation

What we’re looking for
Applicants should have a current PTCB (Pharmacy Technician Certification Board) certification or Licensed Practical Nurse and possess a High School graduate or equivalent. Additional requirements include:
Minimum 1 years’ experience working in a pharmacy or administering medications in a healthcare setting
