- Provide physical therapy services in accordance with physician orders and established treatment plans.
- Assess patient needs and develop individualized therapy programs.
- Educate patients and caregivers on treatment goals and home exercise programs.
- Collaborate with physicians, nurses, case managers, and other healthcare professionals to ensure coordinated patient care.
- Maintain accurate and timely clinical documentation.
- Participate in performance improvement initiatives, competency training, and professional development activities.
- Serve as a clinical resource and mentor for students, volunteers, and staff as appropriate.
- Ensure compliance with all state, federal, and accreditation standards related to home health services.
- Zero to one year of related experience required.
- Strong communication, organizational, critical thinking, and problem-solving skills.
- Ability to work independently and effectively manage time in a home health environment.
- Knowledge of home health regulations and patient care standards preferred.
- Must possess a current, valid South Carolina driver's license and maintain automobile insurance as required by law.
- Candidates must be at least 21 years of age to be covered under McLeod Health's fleet vehicle insurance policy.
- Active South Carolina Physical Therapist License required.
- Current American Heart Association Basic Life Support (BLS) Certification required.
