Brentwood Behavioral Healthcare is seeking a dynamic and talented Case Management for our Outpatient department! 

The Case Manager provides professional case management services to support treatment. This role works collaboratively with social workers, therapists, families, and external agencies to ensure continuity of care, effective discharge planning, and compliance with regulatory standards. The Case Manager facilitates communication between the hospital and community resources, coordinates treatment referrals, and supports patients and families throughout the therapeutic process.

Skills and Competencies

• Previous experience in a mental health hospital setting is preferred.
• Strong communication skills, both verbal and written, are essential for effective patient interaction.
• Strong collaborative skills and ability to work effectively with others.
• Ability to exhibit empathy, respect and compassion that involve understanding and care for patients.

• Bachelor's degree required
• Must have valid DL
• Three (3) years related experience required.
