The RN Clinical Manager/Supervisor is responsible for the overall supervision and coordination of clinical services. Coordinates and supervises an interdisciplinary team of staff to assure the continuity of high-quality care to home health patients assigned to the team's area in accordance with physician prescribed plan of care, and all applicable state and federal laws and regulations.
Essential Functions:
- Counsel's employees as needed according to Agency Policy and Procedures.
- Serves as resource for field staff, patients and / or families, physicians and other community and contract agencies.
- Obtains physical, psychosocial, environmental and health behavior assessment data as evidenced by completed admission forms.
- Completes all nursing documentation and forms within the timeframe as established by Agency. Notes changes in patient's condition through on-going assessment as reflected in the progress notes and follow-up assessments.
- Compares assessment data to baseline assessment to monitor patient's progress. Recognizes alteration in function, including life-threatening situation, and intervene and document appropriately. Initiates realistic teaching plan based on identified learning needs of the patient and / or those providing care and documents appropriately.
- Supervises field staff in their delivery of care as prescribed by the established Plan(s) of Care.
- Provides on-the-job training to field staff. Evaluates competency of field staff and provide remediation training as needed.
- Appropriately documents, investigates and resolves patient / employee complaints and occurrences. Provides employee health assessment and medical compliance requirements as requested.
- Responsible for obtaining prior authorizations as required by third party payers.
Education & Experience
- Current RN licensure in state of practice
- Current CPR certification required
- Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation
- Prior management/supervisory experience preferred.
- Knowledge of home care regulations and regulatory requirements is preferred.
