Position Summary
5-50 Group is seeking a highly skilled Mobile Advanced Practice Provider (APRN or Physician Assistant) to deliver proactive, high-quality in-home and facility-based care for high-risk and medically complex patients.
This role is ideal for a clinically independent provider who is comfortable working in the field while leveraging remote patient monitoring, AI-enabled clinical workflows, and physician oversight to deliver coordinated, data-informed care.
The ideal candidate has strong experience in cardiology, pulmonology, urgent care, chronic disease management, post-acute care, geriatrics, or home-based medical services, and is highly proficient in clinical documentation and care coordination.

Compensation
- $112,000 – $140,000 annually, based on experience and qualificationsÂ
- Additional compensation structures may apply based on workload, productivity, and role classification (full-time vs. per-diem)Â
Key Responsibilities
- Conduct in-home and facility-based assessments for high-risk, post-acute, chronic care, workers’ compensation, and medically complex patientsÂ
- Visit skilled nursing facility (SNF) residents in person at least once per month for assigned patientsÂ
- Conduct home visits according to patient tier protocol, including:Â
- Weekly visits (high-risk tier)Â
- Every two weeks (moderate-risk tier)Â
- As-needed visits based on clinical status, escalations, or care coordination needsÂ
- Obtain comprehensive histories, perform physical exams, review medications, and assess clinical riskÂ
- Order, review, and interpret diagnostics (labs, imaging, EKGs, and other tests within scope/protocol)Â
- Develop, implement, and update patient care plans in coordination with physicians, specialists, and care teamsÂ
- Utilize remote monitoring data, clinical dashboards, and care manager escalations to guide timely interventionsÂ
- Respond to patient escalations and clinical calls per organizational protocolsÂ
- Document all clinical findings, decision-making, orders, communications, and follow-up plans accurately and promptlyÂ
- Collaborate with care managers and field staff to ensure continuity of careÂ
- Support clinical education and guidance for field staff as neededÂ
- Participate in quality improvement initiatives and AI-enabled workflow validationÂ
- Contribute to refinement of clinical protocols and technology-driven care processesÂ
Required Qualifications
- Active Florida ARNP/APRN or Physician Assistant (PA) license in good standingÂ
- Minimum 2 years of clinical experience in one or more of the following areas:Â
- ICU / CCUÂ
- Cardiology or PulmonologyÂ
- Emergency Medicine or Urgent CareÂ
- Hospitalist or Internal MedicineÂ
- Geriatrics or complex chronic careÂ
- Home-based or post-acute careÂ
- Experience with high-acuity or medically complex patients strongly preferredÂ
- Prior experience with home visits, telehealth, or remote patient monitoring strongly preferredÂ
- Strong clinical judgment and ability to work independently in the fieldÂ
- Excellent documentation and EMR proficiencyÂ
- Valid driver’s license, reliable transportation, and insuranceÂ
- Ability to travel throughout the assigned South Florida territoryÂ
- Authorized to work in the United StatesÂ
- Professional English communication requiredÂ
Preferred Qualifications
- Bilingual: Spanish strongly preferred; Haitian Creole is a plusÂ
- Experience with:Â
- Cardiopulmonary assessment and monitoringÂ
- EKG interpretation supportÂ
- Medication reconciliation and polypharmacy managementÂ
- IV skills (preferred, not required)Â
- Experience supervising or coordinating care teams, EMTs, MAs, or care managersÂ
- Familiarity with digital health platforms, dashboards, or AI-supported clinical toolsÂ
- Interest in scalable, technology-enabled home healthcare modelsÂ
Technology & Workflow Expectations
- Use StreamCare EMR, dashboards, and remote monitoring platforms for clinical decision-makingÂ
- Integrate patient-generated data and monitoring alerts into care planningÂ
- Ensure documentation is clear, timely, and usable across multidisciplinary teamsÂ
- Validate and interpret AI-supported alerts using clinical judgmentÂ
- Identify workflow gaps and contribute to continuous improvement of care protocolsÂ
- Maintain compliance with physician-supervised practice standards at all timesÂ
