Family First Homecare is urgently seeking Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) who are experienced in pediatric tracheostomy and ventilator care to join our compassionate home health team!

- Provide expert in-home nursing care for pediatric patients requiring tracheostomy and ventilator management

- Trach and ventilator experience is requiredĀ (pediatric home health preferred)
- Active, unencumbered RN or LPN/LVN license in the state of employment
- Compassionate, reliable, and confident working independently

KEY RESPONSIBILITIESĀ
Accountable for providing care that is based on licensure/certification within the scope of practiceĀ Ā
Maintains an updated clinical record on each patient at all times, meeting required deadlines for documentationĀ Ā
Evaluates the effectiveness of nursing service to the patient and family on an ongoing basisĀ Ā
Consults with physicians concerning alterations of patient care plans, documentation of changes in patient care and reports all changes in care to the nurse supervisorĀ Ā
Supports FFHCās mission by mentoring and/or coaching new employees during patient specific orientationsĀ Ā
Educates patients and their caregivers on topics related to patients' care needsĀ Ā
Participates in FFHC Quality Assurance, Performance Improvement program (QAPI) as requestedĀ Ā
Continually strives to improve his/her nursing care skillsĀ Ā
Attends and/or completes all mandatory training/ in-services as assignedĀ Ā
Submits required personnel file documentation in a timely mannerĀ Ā
Adheres to FFHC compliance and ethics expectationsĀ Ā
Maintains patient and family confidentialityĀ Ā
Maintains professional, supportive, and responsive interpersonal communication skillsĀ

KEY DETAILS:
** This role requires travel to patients' homes to deliver care. The work schedule may include varying shift lengths (8, 10, or 12 hours) and may require availability on any day of the week, Sunday through Saturday.
