Job Details

A caregiver in this role works days from 8:30 a.m. – 5:00 p.m.

Responsibilities:

  • Provides care for chronically ill patient population by applying knowledge of stages of human development, ethnic, cultural and spiritual influences.

  • Demonstrates knowledge of oncology diseases and treatments including level of understanding, reactions and expectations of disease process.

  • Completes psychosocial assessments and develops short and long-term care plans based on individual need in a timely manner.

  • Performs ongoing assessment and continued monitoring of treatment goals based on adjustment to chronic illness and change in functional status.

  • Consults with other medical disciplines and staff regarding bio-psychosocial, environmental, ethnic and cultural factors which affect adjustment to chronic illness and its treatment.

  • Educates and develops clinical care planning with patients, families and multidisciplinary team based upon mutually agreed upon goals to enhance, maintain and promote optimal psychosocial functioning throughout chronic illness treatment and its outcomes.

  • Partners with outpatient team, insurance and billing specialists in the implementation of policy related to social work and outpatient issues.

  • Assists with securing financial coverage for treatments and medications consistent with quality patient care.

  • Analyzes and facilitate referrals to community resources and governmental programs including local, national health and welfare agencies including expertise in accessing assistance based on psychosocial need.

  • Provides crisis intervention expertise as necessary including utilization of high risk screening criteria for case findings and outreach activities which may include mental health and chemical and ETOH dependency resources.

  • Initiates multidisciplinary team conferences regarding complex cases including advocacy for and protection of patient's dignity, confidentiality and right and access to care.

  • Provides effective bereavement support, therapeutic counseling and assistance with referrals for Palliative Disease Management, Hospice and end of life issues.

  • Other duties as assigned.

Education:

  • Master's Degree in Social Work Required.

Languages:

  • N/A

Certifications:

  • Licensed Clinical Social Work (LCSW) required.

  • Certification as Breast Patient Navigator in Breast Imaging and Cancer Care (CBPN-IC) preferred if working in Oncology.

Work Experience:

  • 3-5 years of clinical social work and 2 years of hospital Care Management experience preferred.

  • Knowledge of InterQual guidelines preferred.

  • Effective verbal, written and computer skills.

Physical Requirements:

  • Essential Duties: This position requires knowledge of all aspects of patient care including assessing; planning, coordinating, monitoring and evaluating the patients progress through the entire continuum of care.

  • Physical Duties: This position requires frequent walking, sitting, and standing. Frequent talking and listening when giving instructions and explanations. Frequent use of the phone; fax machines, and occasional use of the computer.

  • Working Conditions: The majority of time the employee spends indoors in air-conditioned areas. Potential for exposure to blood and blood borne pathogens is significant. Clinical exposure to patients requires the use of personal protective equipment.

Personal Protective Equipment:

  • This position requires minimal exposure to infectious wastes such as blood and bodily fluids that mandate the wearing of gloves, masks, gowns, and goggles as appropriate.

  • Follows Standard Precautions using personal protective equipment as required for procedures.
