Now Hiring - Advanced Practice Nurse - Bridge Care Burlington County NJ

 

 Salary Range $125,000 to $135,000 a year

 

 

   Advanced Practice Nurse will be responsible for, but are not limited to:
  1. Conduct visits to patients in accordance with local, state and federal regulations.
  2. Communicate visit findings to designated members of the interdisciplinary team.
  3. Facilitate access to appropriate support models of care (i.e., rehabilitation, on-going aggressive treatment).
  4. Consult with other members of interdisciplinary team to contribute to the patient's plan of care.
  5. Prepare and maintain accurate patient records, charts and documents.
  6. Assist in development of clinical practice guidelines/standards in support of quality care as needed.
  7. Ensure collaboration with patients, caregivers, medical directors and members of IDT.
Education:
 Master’s degree in Nursing (MSN) or Doctor of Nursing Practice (DNP) from an accredited program.
 
Professional Experience:
  • Must have a Nurse Practitioner's license in the state of operations
  • Must have 1+ years' experience working within Family Practice, General Practice, Geriatric care or related field,
  • Must have a valid driver's license and a reliable source of transportation
  • Must be willing to travel        
  • Experience in a nursing home, hospital, assisted living or acute rehab space is a plus but not required.
  • Commitment to ongoing professional development and clinical excellence
